Pharmacist instructions for a patient with an Eligible Third Party: Submit the claim to the primary Third Party Payer first, then submit the balance due to Therapy First Plus as a Secondary Payer COB [coordination of benefits] with patient responsibility amount and a valid Other Coverage Code, (e.g., 8). The patient is responsible for the first $5 and the card pays up to the next $70 for NIASPAN. Reimbursement will be received from Therapy First Plus. When you use this card, you are confirming that you have not submitted and will not submit a claim for this prescription for reimbursement under any federal, state or government-funded healthcare program, such as Medicare (including Part D), Medicare Advantage, Medicaid, Medigap, Veterans Affairs, the Department of Defense or TRICARE.

NIASPAN® (niacin extended-release) tablets are a prescription medication used along with diet when a low-cholesterol diet and exercise alone are not enough.
Taking NIASPAN with another cholesterol-lowering medicine (simvastatin) does not reduce heart attacks or strokes more than taking simvastatin alone.
